Transcript/Document Request Instructions
For alumni or students requesting official or unofficial transcript, or document need to print, complete and submit a Document Request Form and turn the form into Student Liaison Office. The following information MUST be provided in the form via fax, email or online:
- Current/Active E-Mail Address
- Full Name at the time of attendance
- Month and Year of Graduation
- Requested Document
- Number of document being requested
- Complete Organization/School Name and Mailing Address where documents are to be sent
- Your Signature
Direct phone and/or e-mail requests are not accepted and do not call the office to request a transcript (request for your transcript here).
Allow 5 business days for processing.

- Latest Report Card/Progress Report
- Diploma Certificate
- Immunization Records
- Any Document on file
