Transcript/Document Request Instructions

For alumni or students requesting official or unofficial transcript,  or document need to print, complete and submit a Document Request Form and turn the form into Student Liaison Office. The following information MUST be provided in the form via fax, email or online:

  • Current/Active E-Mail Address
  • Full Name at the time of attendance
  • Month and Year of Graduation
  • Requested Document
  • Number of document being requested
  • Complete Organization/School Name and Mailing Address where documents are to be sent
  • Your Signature

Direct phone and/or e-mail requests are not accepted and do not call the office to request a transcript (request for your transcript here).

Allow 5 business days for processing.

  • Latest Report Card/Progress Report
  • Diploma Certificate
  • Immunization Records
  • Any Document on file
